How it works

Sildenafil and tadalafil are phosphodiesterase type-5 (PDE5) inhibitors that enhance blood flow to the penis to make an erection easier to get and keep during sexual arousal. When you are sexually stimulated, your body releases nitric oxide, and these medications work by blocking the enzyme that breaks down the signal responsible for relaxing blood vessels. While sildenafil provides fast-acting support, tadalafil’s defining feature is its duration, often lasting considerably longer as a "weekend" treatment. Neither medication acts as an aphrodisiac, meaning they both strictly require natural sexual arousal to function and will not cause an erection on their own.

Dosage & how to take it

Starting dose: We have no recommended starting dose. Please ensure you read through on the risks associated with the combo.
Adjusting: We have no adjustment metrics. If unsure please use our other three medications.
Maximum: 105 mg, and no more than once in any 24-hour period.
With food: Can be taken with or without food; a heavy or high-fat meal may slow how quickly it works.
Grapefruit: Avoid grapefruit and grapefruit juice, which can raise blood levels and side effects.
Duration: As-needed doses can support erections for up to 36 hours.

Important safety information — who should not take it

⚠ Never combine with nitrates

Do not take Sildenafil+Tadalafil if you use any nitrate medicine for chest pain or heart problems (such as nitroglycerin, isosorbide dinitrate or mononitrate), recreational nitrites (“poppers”), or the pulmonary-hypertension medicine riociguat (Adempas). Together they can cause a sudden, life-threatening drop in blood pressure.

Vardenafil can affect heart rhythm (QT interval), so it should be avoided if you have certain heart-rhythm conditions or take some antiarrhythmic medicines. Tell the prescriber if you take alpha-blockers , or certain antifungal, HIV or antibiotic medicines (CYP3A4 inhibitors), or have heart disease, low or high blood pressure, severe liver or kidney problems, a recent stroke or heart attack, or a history of certain eye conditions. Vardenafil is for adults (18+) and is not for women or children.

Seek emergency care for an erection lasting more than 4 hours, sudden loss of vision or hearing, chest pain, or signs of an allergic reaction.

Common side effects

Most are mild and pass quickly. They include headache, facial flushing, a blocked or runny nose, indigestion, dizziness and back pain. Stop and contact a clinician if side effects are severe or persistent.
